Eligibility
The child must be a Florida resident age 21 or younger, meet Medicaid or KidCare financial and sliding-fee criteria, and meet the Children's Medical Services clinical screening requirements or have physician documentation of qualifying conditions.
How to start
Call Children's Medical Services at 855-901-5390 or email the clinical eligibility unit to request an application. Be prepared to submit the signed application, household-income documentation, and proof of insurance or a Medicaid or KidCare denial. Do not assume a service will be paid until it is authorized.
